Rosemary's Before and After Dental Makeover

How did you develop a plan for treating Rosemary's concerns?

We always start with exhaustive records.  We determine the challenges of the patient's bite, teeth and the surrounding structures. We discuss the patient's concerns and goals and set forth a realistic treatment plan that takes into account the patient's budget.  In addition, a  discussion of the procedures involved and the physical and emotional tolerances of the patient. This may include fears and length of treatments.
Was her treatment out of the ordinary?

The treatment was extraordinary in that it involved rather large space between the front teeth. We discussed all options from orthodontics to close the space to going directly to the restorations (i.e. Porcelain veneers and crowns).  Consultation with our ceramics lab prior to treatment, which included a diagnostic wax up (i.e. a wax mock-up of the final restorations), revealed that a satisfactory result was possible without the use of orthodontic treatment.  This meant that treatment could be completed in several weeks instead of several months or a year. Significant savings were possible by eliminating the orthodontic component.

Michelle's Dental Makeover!

Dr. Steven Millman explains Michelle's treatment plan....

Michelle's case is an excellent example of how we can use porcelain veneers to correct a person smile instantly without orthodontic treatment. 

The patient was very concerned with the symmetry of her smile and her mid-line being off. She also did not like the anatomical shape of her teeth.

A treatment plan was developed with the help of our lab technician Jonathan Murisac and our periodontist Dr. Dana Levy. Prior to treatment we created a wax mock-up of her smile in order to correct her concerns. Once the patient was able to visualize the changes we could  make to her smile, she accepted going ahead with this treatment.

The transformation was completed over a month of treatment. We started off by placing temporary veneers to correct the position of the gingival tissue and to create a symmetric smile. Once the patient was satisfied with the cosmetic result we then replaced the temporaries was permanent veneers.

There are many potential patients that could benefit from this type of treatment. This is a relatively non-invasive treatment that can correct crooked misaligned teeth without orthodontic treatment. As you can see by the photos the cosmetic result was excellent. Both myself and the patient extremely pleased with the final results. 




The beautiful porcelain veneers can be easily maintained with routine brushing and flossing. We also highly recommend six month cleaning at the dental office with a skilled hygienist.

1.     What made you decide to undergo these procedures?  Saw an orthodontist and due to the crowding of my teeth and the extended years of work that needed to be done they didn’t feel it was an good option for me at my age and I would not get the result I was looking for since I was missing the canine tooth. 

2.     Why did you choose Dentistry on Dundas/Dr. Millman?  He is my family dentist so didn’t feel the need to look elsewhere.

3.     What were your procedures like?  It was definitely time consuming, however this was due to my specific case as we were turning another tooth to appear to be a canine. Other than that, there was nothing intolerable. 

4.     How do you feel about your results?  Anyone who sees them think they look perfect and have had comments on how perfect they look from people who don’t know I had them done.

Barbara's Dental Makeover!

 

Barbara underwent several procedures to achieve her new smile. This included root canal therapy, surgical crown lengthening of upper anteriors and full crown and bridgework on upper teeth.

What was her chief concern about her teeth?
Barbara's previous bridgework was failing. One of the teeth that supported her bridge were breaking down and needed adjunctive gum surgery to make them candidates for support for the new prosthesis. Her case presented some challenges as she had also lost some posterior support. She would require restoration of her bite to complete her treatment. Ideally implants would be the best option but she opted at this time for partial dentures until implants could be considered.

Why are implants the better option in many cases?
implants are now the treatment of choice in most cases when it comes to the replacement of lost teeth. Although they are biologically different from teeth, they act like teeth in many ways. Aesthetically they are usually the best way to mimic what nature originally gave us. They can stand alone as individual teeth, something a bridge or partial denture can't.  Implants do not affect the adjacent teeth, unlike other alternatives that rely on adjacent teeth for fixation or support. However, implants do come at a higher cost but that is only in the short term. Over the long haul, properly integrated and maintained implants can be a bargain when you consider the costs associated with maintaining and replacing the other alternatives.

What further treatment will Barbara need as she moves forward?
Post treatment care is always the key to a successful restoration. maintenance at home as well as frequent visits to the dentist for examination and cleaning of the restoration and all of her teeth will help ensure many years of smiles.  in addition, the sooner implants are placed in the posterior dentition the better.  for it's the posterior teeth that protect the anterior teeth and the smile.

A Career in Dentistry?....

One of our patients is a grade 12 student at Donald A. Wilson Secondary School.  As part of a French project she is required to research a profession in which she is interested in, as you might have guessed the profession she chose is dentistry.  She presented us with few questions regarding our profession for her dentist, Dr. Librach, to answer. Here is what he said...



1.      What is the most difficult part of your career? Explain.

The most difficult part of dentistry is keeping up with technology.  The practice of dentistry has changed so much since I graduated in 1987.  Although the principles of good dentistry have not changed, the methodology advances have changed dramatically and will do so exponentially in the coming years.  At Dentistry on Dundas we continue to invest in the latest technology that gives our patients the highest quality dentistry.  With that commitment comes a responsibility to stay on top of the latest developments and to chose for our patients those technologies that will offer them the greatest benefit.   It can be a daunting task but, on the other hand, it continues to make dentistry an exciting profession.

2.      What is the best part of your career? Explain.

Working with great people. Firstly, our staff at Dentistry on Dundas are some of the best people I have ever met.  They are compassionate, competent and fun to work with.  They are as important to my practice as having the best equipment and knowledge.  They are an integral part of what attracts patients to our practice.  Secondly, our patients, many who have been with us since our start in 1989, make this career second to none.  There isn’t a day that goes by that I do not feel blessed that these people have put their confidence in me and our staff.  The interactions with our patients makes me excited to come to work every day.

3.      How competitive was the job market when you started working versus today? 

Quite a change has occurred over the last 27 years.  There are definitely more options for patients when it comes to who they see for their dental care.  When I graduated dental offices were fewer especially in the Durham region.  In addition, denturists (those that only make dental prosthesis like false teeth) are licensed to practice in Ontario.  Finally, Dental Hygienists  are now able to practice independent of the supervision of a licensed dentist.   That being said, I welcome the competition.  It keeps your business sharp and challenges you to be the best in your field.  At Dentistry on Dundas we continue to be successful in a very competitive market by offering our patients great dentistry in a comforting atmosphere.

4.      What advice would you give to someone starting out in orthodontics?

There are so many facets of dentistry that you should keep an open mind when entering dental school.  Orthodontics is a great profession but so is Endodontics (root canal therapy), Oral and Maxillofacial Surgery ( jaw surgery and complicated extractions), Oral Radiology, Periodontics (implants, gum surgery & care), Paedodontics (oral care for children), Oral Anaesthesiology (dentistry while asleep) and Prosthodontics (the restoration of complicated oral cavity collapse).

And lets not forget General Dentistry that allows you to perform any, some or all of these disciplines as you expand your knowledge in these fields. In addition, comprehensive treatment planning requires multiple disciplines.  At Dentistry on Dundas we have specialists in most of these fields that work as a team “quarterbacked” by a treatment plan set out by the General Practitioner.  Having said that, Orthodontics is a great specialty in dentistry and I would only encourage some one to continue to follow that path if that is where their interests lie.  Any path you chose you should always keep in mind that you serve the patient.  The rewards of this profession will come on its own if you treat your patients with respect, compassion, empathy and keep their best interests a focal point.

Why was (patient) a good candidate for these procedures?
Carol was a great candidate for this dental makeover.  She always has a great attitude and she needed a smile to compliment her personality.  She is out going and her personality lights up a room.  Now she has a smile to match.  In addition carol knows what it takes to maintain her beautiful smile. Great home care and regular visits to the dentist will ensure the longevity of her restorations.

Was her treatment out of the ordinary?
Although Carol's treatment was not extraordinary in the complexity of the restoration, it was important to Carol that her smile was perfect.  As always, the attention to detail and symmetry was foremost when treatment planning her case. When the patients expectations and commitment to the outcome match those of the doctor's, the results can be amazing. It was my pleasure to work with Carol.

Small treatment creates BIG results!

Penny underwent a few small dental treatments to achieve her new smile. This included teeth bleaching, two new anterior crowns and cosmetic bonding.
 


Why was Penny a good candidate for these procedures?
Penny was a great candidate for her transformation. She was actually an extremely apprehensive patient that was nervous about most routine dental visits. Once she understood how standard the procedures were and how relatively quick they would be completed, she became more relaxed with each visit. She realized that a drastic result could be attained with ease, thus, a great case!

How did you develop a plan for treating Penny's concerns?
In this particular case, during the initial examination, it was determined that a cleaning, some fillings and whitening were required before the front crowns were done.The first four appointments addressed the fillings and cleaning. These "easy" appointments were important to gradually help the patient relax with each session, and to create a healthy oral  environment. The whitening was done at home at  by the patient at their convienience. All it took to change the front teeth were the last two appointments.

Is there an ideal candidate for these procedures?
The ideal candidate for these procedures, or any for that matter, is a motivated individual who is committed to improving their oral care and esthetics. Everyone and every case is different. It may be few or many appoinments, however, there are always options where most issues can be addressed and improved upon. No matter how busy, the treatment can always be accomodated  to any schedule.
 
Are there any at-home steps patients should follow after procedures like these?
It is extremely important to maintain the health of any new restorations such as these crowns that were done on this patient. Home care and follow up cleanings are essential. Oral hygene instruction are reviewed and often, the patient is sent home with some special cleaning tools. Occasionally, a patient is asked to return one week later after a "large" case with many crowns, for example. This is to ensure the patient is satisfied, comfortable, and the tissues are forming to the new restorations appropriately.

Martine's smile makeover!

Martine underwent several months of dental treatment to achieve her new smile. This included several root canals, surgical crown lengthening of upper anteriors, full crowns on all upper teeth and 5 dental implants to replace missing teeth in her lower jaw.


Why was Martine a good candidate for these procedures?

Martine was a good candidate because she was motivated. She understood that she would get immediate temporary results but the final permanent restorations were highly technique sensitive and would require several months to achieve.

How did you develop a plan for treating Martine's concerns?

Although Martine’s initial concern was the overall appearance of her smile, we had to make sure that a beautiful smile starts with function. After all, she needs to be comfortable eating. Treatment plans always start with the evaluation of the supporting structures, the bone and the gums. These form the foundation of a long lasting result. Once the gums, bone and the bite have past the test, a smile can be constructed that is natural and beautiful. In the mean time we were able to get Martine back her smile immediately with a temporary restoration that we could use to develop her long lasting smile.

Was her treatment out of the ordinary?

Absolutely! Although we are not strangers to complex cases such as Martine’s, each case of this magnitude has it’s own challenges. Any treatment that takes several months requires several disciplines in dentistry. In this case, the expertise of several specialists was called into play so the coordination of Martine’s primary care dentist was very important. This is made easier when all of the specialists are housed under one roof as they are at Dentistry on Dundas.

How long did Martine's transformation take?

The initial transformation was almost immediate but several treatments were required over a year period till we could reach our final goals.

What steps do these procedures involve?

Treatment planning, Treatment planning, Treatment planning! …..followed by a series of logical treatment steps keeping in mind that twists and turns may arise that require adaption and flexibility.

Is there an ideal candidate for these procedures?

Martine and patients like her that are motivated are ideal for this type of treatment. They understand the insurance plans do not dictate what treatment is required, the patient and their dental health does.

Are there any at-home steps patients should follow after procedures like these?
As with anything manmade, how you take care of it plays a huge factor on any restorations longevity. After all, Gods work didn’t last forever either. So home care, regular visits to the dental office, paying attention to small problems quickly as they arise and protecting the restorations at night through use of a nightguard will go along way to extending the life and beauty of the smile we have created.
